Marijuana Isn't as Harmless as Many People Think: What the Research Really Says

Public attitudes toward marijuana have changed dramatically over the past few decades. As more states and countries have legalized cannabis for medical or recreational use, many people have begun to view it as a relatively harmless substance. Surveys consistently show that perceived risk has declined, especially among adolescents and young adults. Some even argue that because marijuana is "natural," it must be safe.

The scientific evidence paints a more nuanced picture. Marijuana is not as dangerous as some drugs, such as heroin or methamphetamine, and it has legitimate medical applications for certain conditions. However, it is also far from risk-free. Decades of research have linked frequent cannabis use—particularly heavy use beginning during adolescence—to problems involving memory, attention, motivation, mental health, and the development of cannabis use disorder. The takeaway is not that marijuana is universally harmful, but that many of its risks are commonly underestimated.

Why Has Marijuana's Reputation Changed?

​

Much of marijuana's improved public image is understandable. Medical cannabis has helped some patients manage chronic pain, chemotherapy-related nausea, muscle spasticity associated with multiple sclerosis, and certain forms of epilepsy. As legal restrictions have loosened, researchers have also gained a better understanding of both the therapeutic potential and the limitations of cannabis-based medicines.

At the same time, legalization has often been interpreted by the public as proof that marijuana is harmless. History shows that legality and safety are not the same thing. Alcohol and tobacco are both legal for adults in many countries, yet they remain major contributors to preventable disease and death. Likewise, marijuana's legal status says little about its effects on the brain or its potential to create problems for some users.

Marijuana Affects the Brain

​

The primary psychoactive ingredient in cannabis, tetrahydrocannabinol (THC), works by interacting with the brain's endocannabinoid system. This system helps regulate learning, memory, attention, reward, mood, appetite, pain perception, and many other important functions. Because cannabinoid receptors are found throughout the brain—especially in areas involved in memory, decision-making, and executive functioning—THC can temporarily interfere with these mental processes. During intoxication, many users experience slower reaction times, poorer attention, impaired short-term memory, and difficulty concentrating. These effects help explain why driving or operating machinery after using cannabis increases the risk of accidents.

What Happens With Long-Term Use?

​

One common misconception is that once the immediate "high" wears off, the brain returns completely to normal. Research suggests the answer is more complicated. A major review published in Nature Reviews Disease Primers by Wayne Hall, Jason Connor, Bernard Le Foll, and colleagues examined decades of evidence on cannabis use and cannabis use disorder. The authors concluded that while many people use cannabis without developing major problems, regular and heavy use is associated with measurable risks. These include impairments in attention, learning, and memory, an increased likelihood of developing cannabis use disorder, and elevated risks for certain mental health problems in vulnerable individuals. The review also emphasized that today's cannabis products often contain substantially higher concentrations of THC than those available several decades ago, potentially increasing these risks.

Other systematic reviews have reached similar conclusions. Although the average cognitive effects are generally modest for occasional users, heavier and more persistent use is associated with poorer performance on tests of memory, attention, processing speed, and executive functioning. Some of these changes improve after prolonged abstinence, while others may persist longer, particularly among individuals who began using heavily during adolescence.

Adolescence Is a Particularly Sensitive Time

​

One of the strongest findings in cannabis research concerns when people begin using. The adolescent brain continues developing well into the mid-twenties, especially regions responsible for planning, impulse control, decision-making, and self-regulation. Because the endocannabinoid system helps guide normal brain development, introducing large amounts of THC during this period may interfere with these developmental processes.

Longitudinal studies suggest that individuals who begin using cannabis heavily during adolescence are more likely to experience persistent difficulties with attention, memory, academic performance, and executive functioning than those who begin later in adulthood. Researchers continue to debate exactly how large these effects are and the extent to which other factors contribute, but adolescence consistently emerges as the period of greatest vulnerability.

Marijuana Can Be Addictive

​

Another widespread myth is that marijuana is not addictive. While cannabis does not produce the severe physical withdrawal associated with some drugs, it can lead to cannabis use disorder (CUD). Individuals with CUD may find it difficult to cut back despite negative consequences, spend increasing amounts of time obtaining or using cannabis, experience cravings, and continue using even when it interferes with work, school, or relationships.

Current estimates suggest that roughly one in ten people who use cannabis will develop cannabis use disorder, with the risk increasing substantially among daily users and those who begin during adolescence. Withdrawal symptoms—including irritability, sleep disturbances, decreased appetite, anxiety, and restlessness—are now well recognized by clinicians.

Mental Health Risks

​

Cannabis does not affect everyone in the same way. For many people, occasional use produces relaxation or mild euphoria. Others experience anxiety, panic attacks, or paranoia, particularly after consuming high-potency products or large doses of THC.

Research also indicates that frequent cannabis use is associated with an increased risk of psychotic disorders among individuals with certain genetic or environmental vulnerabilities. Importantly, researchers do not argue that cannabis causes schizophrenia in everyone who uses it. Rather, cannabis appears capable of increasing risk among susceptible individuals, especially when heavy use begins early in life.

A Balanced Perspective

​

None of this means marijuana should be viewed as uniquely dangerous or that every user will experience serious problems.

Many adults use cannabis occasionally without developing dependence or long-term cognitive difficulties. Medical cannabis has demonstrated benefits for some health conditions, and ongoing research continues to explore additional therapeutic uses. At the same time, scientific evidence clearly shows that marijuana carries genuine risks—particularly for adolescents, heavy users, frequent users, and individuals with certain mental health vulnerabilities. The healthiest approach is neither fear nor denial. It is informed decision-making based on scientific evidence rather than myths from either side of the debate.

The Bottom Line

​

Public opinion has shifted rapidly toward viewing marijuana as a low-risk substance. While cannabis is certainly less harmful than some illicit drugs, it is not harmless. Heavy or frequent use can affect learning, memory, attention, motivation, and mental health, and some users develop cannabis use disorder. The greatest risks appear among individuals who begin using regularly during adolescence, when the brain is still developing. Science rarely supports simple slogans. Marijuana is neither the terrifying menace portrayed in early anti-drug campaigns nor the completely harmless substance sometimes portrayed on social media. Like many psychoactive substances, it has both potential benefits and real risks. Understanding both sides allows people to make more informed decisions about its use.
